Dementia Care Connects - Community Outreach Hubs

Tackling loneliness and isolation, improving older people’s independence and improving both emotional and physical wellbeing remains a high priority for our organisation. In this respect, through our previous work around Community Outreach Hubs and the piloting of dementia friendly activities, we created our Dementia Care Connects Community Outreach Hubs.

The project which is supported by the West Glamorgan Regional Integrated Fund aims to help ensure that those at increased risk of developing dementia, those with an early dementia diagnosis and/or those that are experiencing the highest levels of isolation, loneliness, disadvantage and deprivation have direct access to a holistic community level prevention and early intervention activities programme.  Activities are fun and user led as we aim to create a positive experience of ageing, with local residents feeling involved, appreciated, more confident and active within their community.

Our Dementia Care Connects Hubs are delivered in accessible community venues and aim to reduce loneliness and isolation, develop social networks and assist service users to remain as independent as possible with less need for statutory services intervention. A wide range of social and leisure activities including physical activity sessions, creative activities, reminiscence work, health promotion sessions, digital inclusion, excursions and musical therapy are provided and contribute to improving emotional and physical well-being, whilst helping to facilitate attendees to lead a healthier, more active and independent life. 

 Currently, our weekly Dementia Care Connects Outreach Hubs take place at the following venues:
  • Neath Community Centre, Orchard Street - Monday's 10:00 - 12:00
  • Bertha Road Community Centre, Margam - Tuesday's  10:30 - 12:30
  • Pontardawe Library - Thursday's 13:00 - 15:00
Independent Information and Advice
The Dementia Care Connects Outreach Hubs also ensure that attendees have direct access to a confidential and independent information and advice service. This service covers topics including welfare benefits, health, housing, energy efficiency, employment, consumer, leisure and learning. Support may also be provided on complex issues such as future living arrangements and care home fees.
