Hospital  to Home Service

Many older people live alone or with an elderly partner or relative. After a stay in hospital the transition from hospital ward to home environment can be traumatic and additional support may be needed to readjust. 

Age Connects Neath Port Talbot's flexible Hospital to Home Service can help to get people back into their homes following a period in hospital. This help is available to people who live alone or will be alone during the day, those who live with a carer who may need support themselves or who are frail, housebound, have poor mobility and/or limited support networks in place. 

Following discharge from hospital, the service will provide practical, emotional and liaison support to help individuals regain their independence and reduce any risk of readmission to hospital. On initial contact, an assessment will be carried out to identify immediate and short-term support needs and help to avoid duplication by identifying other agencies or services involved.  Services offered within this Hospital to Home Service may include, but not be limited to the following:

• Transport home
• Ensuring utilities are in working order and access to a working telephone
• Ensuring basic provisions such as bread, milk, fruit etc. are in the home
• If appropriate, assist with preparation of light meal/snack 
• Food delivery
• Collecting prescriptions/medication
• Changing of bed linen
• Reassurance and emotional support
• Companionship
• Independent information, advice and welfare benefits support
• Support to carers
• Ensure that relevant contact details are easily accessible (after care details, and emergency details etc.) 
• Onward referral to other appropriate services and liaison support with family and partner organisations/services. 

The service will operate from 08:30 – 16:30 Monday to Friday and is available to people living throughout Neath Port Talbot. To find out more information about this service, please get in touch.
